Recently finished up a Space Wolves Lone Wolf for my brother. No mods, straight out the Wolf Guard Terminator box.

Just to take it a step further, I recommend hitting the gold with a brown wash and giving it a quick highlight.

On a side note, those Space Hulk terminators sort of show that even GW can end up with models with their arms sticking out of the sides of their heads.

 

Is it so hard to sculpt a human armature, and then model the armour around that?

Well you also have to think about how its going to sit on the sprue, the limitations of armour that they are wearing and the possability that they dont have a mock up of the armours and guys to pose in them while they sculpt.
