Malatox
I doubt it since Librarians were being introduced DURING the Great Crusade and were temporarily decreed banned. I would say it's just like the Land-Raider variants and as time progressed situations arises that dictated the entombment of a Librarian hero into a Dreadnought... Just my $0.02 ^_^ Malatox

Hengist Ironfang
Librarians were outlawed from nikea to the last days of the siege, according to collected visions the Emperor rescinded his ban on the golden throne after his battle with Horus iirc, so I suppose it would be possible that some were put into sarcophagi during the crusade and maybe some were created during the cleansing, its also possible that a space marine veteran who was a banned librarian (fighting but not using his powers) fell during the Heresy, was placed in a dread and was modified later to allow the use of his powers.
