Taz
Like I mentioned above, this may have been already discussed, but I opened up my Space Hulk game for the first time in a while (I don't have nearly enough occasions to play, but se la vie) and I was thumbing through the missions booklet and did a double-take at the name of the Libraian, Brother Calistarius. A double-check of our codex confirms that it's letter-for-letter the same name Mephiston had before the second Armageddon war. A wiki-search later I made sure that the events of the Sin of Damnation takes place before the second war for Armageddon. I think that given the longetivity of space marines in general and BA in specific it's entirely possible that the Librarian model in Space Hulk is Mephiston before he becomes Mephiston. Just thought it would be an interesting little funfact, assuming of course I'm not the last guy to figure this all out.

BOBMAKENZIE
You missed the most important part about space hulk! We had two chapter masters whilst it was going on. But yeah Mephy was there before he was Mephy ;)
AbusePuppy
There's a reason that Librarian is rockin' awesome in the board game.
