With all the fantastic elements in the HH series and you can't assume there is a drug or technology that keeps these gladiators alive, or that Angron's Nails were modified by their creators, or the gladiators weren't entirely human? Probably comes with a host of side effects (paranoia, violent mania, impulse control problems, delusions, etc) but since the whole purpose is to make angry superstrong killers... I am pretty sure that Lorgar states directly in Betrayer that Angron cannot sleep. In other words he has really bad (mechanically induced) insomnia. Which makes sense, he's alive but clearly years of torture (sleep deprivation) has had a significant effect on his character and behaviour.
