The idea is that he has been possesed by a nurgle daemon that has bloated a once normal sized human beyond recognition and has driven him mad with pain. he was chained down but he pulled himself free and ripped up the tackle that was holding him its particularly hard to see some of the detail on the unprimed model as the plastic is a few different colours.
