I don't know the source, but I have seen a pic where the left arm, the arm with the Narthecium, was red as well as that half of the helm, the other arm and helm half white, rest normal Salamander scheme. I do recall at one point seeing a pic of one with the right side red, opposite of the Nathecium, like our_baz mentioned. Yet again I have seen some painted as mostly normal scheme with a white helm and white right shoulder with the normal red helix.

 

I also read an article that seemed to draw things from the old 2nd Edition Space Marine Codex (talked about referring to the Codex for Chaplain Xavier, beautiful model btw) that said legend has is it an old Apothecary refused to wash the blood of his Narthecium arm, so it became tradition to paint it red. This article was in no way whatsoever associated with GW or its affiliates, and sadly I do not have that old Codex to check if it actually says that.

 

Back to your question, I would go with the red arm/half head, that seems more fluffy, and I would just choose which side you think looks best.

In the old RT era painting guide they had a triple color scheme, like so:

The 'Eavy Metal Salamanders Command Squad doesn't use that scheme. Their Apothecary is painted like a regular Salamander except: white helmet with a green laurel painted on it, white/red inset on the Apothecary shoulder pad (regular green trim), and white narthecium. It's on p.111 of the old codex.

 

It's a cool scheme and I can't see any reason you shouldn't continue to use it, but be warned GW seem to have abandoned it.
