Standard WH40K uses 2", so it will remain 2" here.

 

Remember, the intent is not to create a vastly different set of rules. That's what the Heralds of Ruin, Killstorm, and other rules do (and well).

 

These rules are meant to enhance the official Kill Team rules while remaining as faithful as possible to the standard WH40K rules. The point is to allow players to transition between the two styles without a significant learning curve with regard to rules.

The issue here is the as possible bit. If you keep it at 2" it may well just make the exercise pointless, because ultimately, kill team is a very different game, with different assumptions. It might work, but I have a feeling it will just be frustrating.

Realistically, though, if we were trying to represent the Apothecary use his narthecium, the rules would require that he be in base contact with the other friendly model.

I always figured that the FNP represented the space marines fighting through wounds they would normally draw back from, because they know medical attention will be immediate/ gene seed recovery will be immediate, rather than because they are literally being treated in between gunshots, rather than just at lulls.
