Hellbrute Power Scourge

The weapon profile says "each time the bearer attacks it can make 3 additional attacks with this weapon". So if I have a hellbrute equipped with a hellbrute fist and a power scourge, do all 7 attacks have to be with the scourge or is it 4 with the fist and then only 3 with the scourge?

This, normally. Compare and contrast with the typical wording for Chainswords or Ork Choppas. You actually have the option of splitting up the default attacks between the weapons as you so see fit, provided you announce this before you roll the dice. So, you could throw two helbrute punches and flail the scourge five times, for example.
