Forgebreaker, the hammer made by Fulgrim for Ferrus and later presented to Perturabo, was, according to the english Lexicanum, retrieved by a terminator of the Clan Avernii sometime after the heresy and returned to Medusa. However, this is the only place I have found that bit of fluff. The 40k wikia doesn't have it and neither does the German Lexicanum. Matter of fact, the English Lexicanum does not cite from where that bit of fluff comes from and quite frankly, it sounds sketchy at best.
