Unless you have some serious sculpting skills the two most likely options of the phobos librarian or Indomitus ancient have already been mentioned. 

 

In my head, a cool combo would be the Indomitus ancient with the arms from the Dark Vengeance company master to recreate somewhat the pose of the old Ezekiel mini.

If you can still find one, both arms and the sword of that company master are one piece that snaps into the body, but the possible worry of putting them on a primaris figure is that the scale difference might have the arms being too small. 

take zachariah, cut up a shoulderpad to make the headpiece librarians are so famous fore and run some wires off it to the backpack, im a big fan of using telecomm wires and just carving them up for scraps. for a sword theres a whole variety YOU could use but ezekiels is 2 handed so youll need to get a long handle regardless and youll need to replace the plasma with a bolt pistol.

 

you can even keep zachariahs head and just green stuff in a mouth cover, OR YOU could borrow the head of iron father ferrios and just green stuff on a hood since weve already come this far for the sake of the premium primaris non phobos experience

if YOU want to add the banner theres a few ways to get that done YOU could make your own freehand one onto an old tactical marine back flag, or get one from elsewhere

 

the dark angels upgrade sprue has a perfectly good book on it, on the primaris kit, and on the veterans kit, and on the older normal marine upgrade kit

Use tigirius body, remove all the ultramarines symbol, add the head of the phobos librarian and the arms of the classical primaris librarian with a DA shoulder pads  (and maybe the back banner of current model of ezekiel),  et voila you got a guy in primaris armor, with robes, the ornament of a chief librarian  and the correct gear (also green stuff might be useful, especially for the head)
