So allot of years ago at the end of 6th I started my Deathwatch army pre codex using emulate marine rules, mid 7th when having issues sourcing rarer shoulder pads that are hard or too complex to free hand I asked a seller on shapeways which made chapter pads in sets if I sent him some chapter images and names could he reproduce them, now that well known seller makes allot more but some of them he no longer does, I no longer have access to the email address I used to contact them so I can’t check the messages and there’s a few pads I’m not 100% on, some like flesh tearers are made by gw now but some of them are allot rarer chapters, the below picture is on the ones I’m not 100% on, if you could help me identify them so I can locate the chapter paint scheme that would be a huge help.

Im fairly certain the hourglass is star phantoms, the chalice I think is blood drinkers, the wavey fire bird I think is fire hawks, but not sure on the rest.

sadly with age they have become sticky and refrosted, going to reclean them soon.

Many Chapter badges/icons are not unique, with multiple Chapters often using the same basic design but with different colors or slight variations. The most ready examples are the Imperial Fists and Crimson Fists Chapters, both of which use a clenched fist (the Imperial Fists using black and the Crimson Fists using red). You can very easily use any/all of the above Chapters. Alternately, you can use some with different colors to create new Chapters.

The "variations" row is for Chapters whose icons include the elements that show up on the shoulder pads you've shown, but which also include some other elements. For example, the Absolvers Chapter icon is a crimson chalice with a white aquila in the upper portion of the chalice. Many Chapters allow for variant forms of their icons to be used, so a Soul Drinkers battle brother might not include the laurel or they may add personal elements to the version of the icon they bear upon their left pauldron. I've generally restricted these entries to Chapters whose icons feature the sculpted element(s) in roughly the same size and position as that which you've shown. There are many other Chapters whose variant badge forms might also be included, though their icons usually have the sculpted elements sized or positioned differently. For example, the Emperor's Swords Chapter features a large jawless skull such as that on the shoulder pad in the top right, but the skull is slightly smaller and there is a pair of crossed swords behind it. Similarly, the Emperor's Shields Chapter also features a jawless skull. That skull is smaller and is depicted as the boss of a large shield.

While I'd usually agree with your lists, it looks to me like the skull on that third pad has its mandible/jaw, just like the Doom Legion's icon, rather than the jawless skull we're so used to see...
It's difficult to make out. I'll defer to Duke Danse Macabre's ability to more clearly make the details out. If the skull has the mandible, then I agree that the Doom Legion is the most likely. If the skull doesn't have the mandible, the Chapters I listed are more likely.

The skull has no lower jaw, trying to trace pictures of transparent resin which has started to frost in some areas is difficult, this was the best of 30 pictures on the clearest pads.

I’m aware some chapters reuse allot of symbols but that has really narrowed it down, I try not to go too far down the rabbit hole with less known chapters, just ordered an extra 50 pads to help bulk out numbers when combined with my old FW pad collection.
