Some kind of power maul. The Ravenwing Black Knights kit comes with a pretty gnarly looking one and I can see this being something similar.

My first thought is some sort of Votann Concussion Maul (although they already have something with that name). 

Votann concussion melee all features similar flat circular parts on the striking surface.

Reminded me of bullgryn mauls or the traitor ogryns club - so maybe heresy era ogryns? They got a mention in a recent heresy article.
