I wanted to post this here too. I just finished my Captain for my 3rd Co. Force. Took forever, but it came out good.

Looks like an Iron Warriors helmet, Cato Sicarius body, Company Champion sword arm, and the shield from the Masters of the Chapter set.

You are entirely correct. All bits accounted for.

 

As for the helmet, it is a Dire Avengers crest GS onto a Iron Warriors helmet.
