Nothing quite so dramatic as some of these, personally...

As far as lost or forgotten items, I still pine for my second Defiler which disappeared when I moved house one time...

For accidents my worst was probably a glue bottle explosion from letting a big fishing case full of odds and ends sit in a summer car for too long - again, during a house move. Luckily I had the presence of mind to put the glue bottle in a little hollow of one corner in the top of the case, but my Wraithlord (the biggest Wraith unit then in production) was up there with his arm-mounted falcon pulse laser pointing too close to the glue pool and I think the tip of that gun is still glued down in there. The Model sat languishing through ~3 editions of 40k until I re-painted for new 'Dex (and still not based, lol):

I was once happily hobbying in my pants and dropped a space marine helmet. Didn't hear it bounce, and couldn't find it on the chair. Strange, I thought, but moved on. 

Turns out I'd super glued it to my inner thigh, but didn't figure it out until I was at work the next day.

I sliced my middle finger down to the bone as a kid trynna cut a plastic marine in half with a blunt pen knife. The scar is still really visible and I did that back in 94-95, I still remember the feeling of the blade slicing through even after all these years. 

 

I also have a very funny scar on my butt because I put my hobby knife down on the sofa and went to get a drink, forgot the knife was there when I sat down...
